Objective: To present the clinical characteristics, management and treatment of an oroantral communication through a clinical case to demonstrate the resolution of this pathological condition at the dental and surgical level. Method: Descriptive qualitative research through clinical case report and bibliographic review of articles published in high-impact journals in English and Spanish from 2016 to 2021. Results: A case of a 38-year-old male patient with oroantral communication following failed extraction of the upper right first molar, with displacement of root remnant to the maxillary sinus and chronic infection is presented. Treatment included antibiotic therapy, surgical cleaning, closure with buccal flap and subsequent maxillary sinus evacuation. Conclusions: Oroantral communication requires timely diagnosis and specialized treatment. Successful management depends on infection elimination, adequate tissue cleaning and appropriate selection of surgical closure technique.
