Effectiveness of telemedicine use in the follow-up of type 2 diabetes mellitus in Andean communities [Efectividad del uso de telemedicina en seguimiento de diabetes mellitus tipo 2 en comunidades andinas]

Objective: To identify the effectiveness of telemedicine use in the clinical follow-up of patients with type 2 diabetes mellitus in rural Andean communities of Ecuador. Method: Systematic review following PRISMA guidelines. Studies published between 2020-2025 in Spanish or English, evaluating telemedicine interventions in rural contexts, were included. Search conducted in PubMed, Scopus, SciELO, LILACS, and Google Scholar. Of 58 initial studies, 19 met inclusion criteria. Rayyan software was used for blinded peer selection. Results and conclusion: Telemedicine demonstrated HbA1c reduction between 0.5%-1.2%, improvement in therapeutic adherence, and high patient satisfaction. The most effective tools included remote monitoring, video calls, mobile apps, and educational SMS. Results were comparable or superior to in-person follow-up. Telemedicine is confirmed to be effective and adaptable to Ecuadorian Andean communities, requiring cultural adaptation, consideration of local language, and strengthening of technological infrastructure for sustainable implementation.
