Abstract

Objective: Describe the endodontic management of a patient with Down syndrome who presented with pulp necrosis and symptomatic apical periodontitis. Method: Clinical case report of an 18-year-old male patient with Down syndrome treated for extensive caries in tooth #36. A complete medical history, clinical examination, diagnostic tests and radiographic analysis were performed. Treatment included necropulpectomy with RACE EVO rotary instrumentation, ultrasound-activated irrigation, and thermoplastic filling. Results: The patient presented characteristic oral manifestations: microdontia, conical teeth, skeletal class III, macroglossia, ogival palate, and generalised periodontal disease. Endodontic treatment was completed using appropriate behavioural management techniques and a multidisciplinary approach. The prognosis was guarded due to severe periodontal involvement. The 6-month follow-up showed favourable progress with complementary periodontal management. The case highlights the importance of early preventive care and specialised training in this population.
