Objective: analyze community-based interventions for the prevention of chronic diseases. Method: PRISMA systematic review. Thirty articles were processed as the final population, located in databases such as: Scopus, PubMed, Web of Science. Results and conclusion: Community interventions represent a fundamental public health strategy for the prevention and management of chronic and communicable diseases, by promoting healthy lifestyles and reducing risk factors. Comprehensive programs involving schools, families and communities have proven effective in preventing childhood obesity, while culturally adapted strategies and the use of technologies have optimized the control of diseases such as diabetes and hypertension. Likewise, community health workers play a strategic role in strengthening the link between communities and health systems, promoting adherence to services and active participation.
