Objective: To present a clinical case of chronic ischaemic heart disease with homo- and heterocoronary collateral circulation. Method: Clinical case of a 75-year-old male patient with a history of high blood pressure, long-term smoking and exertional angina. Results and conclusion: The chronic total occlusion of the anterior descending artery, partially compensated by insufficient collateral circulation, together with the presence of non-sustained ventricular tachycardia, reflects the anatomical and functional complexity of the clinical picture. Despite failed attempts at recanalisation, individualised pharmacological management, with therapeutic adjustments aimed at improving tolerance and efficacy, allowed the patient to be stabilised.
