Smoking and progressive renal impairment: decreased GFR, proteinuria, and endothelial dysfunction [Tabaquismo y deterioro renal progresivo: disminución de TFG, proteinuria y disfunción endotelial]

Abstract

Objective: To analyse the evidence on smoking as a factor in the progression of kidney damage. Method: A literature review was conducted in PubMed, Scopus, Science Direct, and SciELO of articles published between 2019 and 2024 in Spanish, Portuguese, and English. Results: 78% of smokers had an GFR below 60 ml/min, 65% had elevated proteinuria, and 80% had uncontrolled hypertension. Smokers have a 2.5 times higher risk of accelerated GFR decline. 70% were unaware that smoking affected kidney function. The early biomarkers NGAL, KIM-1, NAG, transferrin, and GM2AP are elevated in smokers. The mechanisms include oxidative stress, endothelial dysfunction, and activation of the renin-angiotensin-aldosterone system. Smoking cessation for six months showed stabilisation of proteinuria and slower progression of kidney disease. Conclusions: Smoking significantly accelerates the progression of chronic kidney disease. Early smoking cessation reduces kidney deterioration and stabilises glomerular function.
