Abstract

Objective: To analyse the impact of paricalcitol in the treatment of secondary hyperparathyroidism in patients with chronic kidney disease, evaluating its efficacy in reducing parathyroid hormone and its effects on cardiovascular complications. Method: A systematic review based on PRISMA methodology was conducted of publications in PubMed, BVS, and SciELO between 2021 and 2025, identifying 552 references, of which 10 were included. Results: Paricalcitol demonstrated effective reduction of parathyroid hormone levels without inducing significant hypercalcaemia or hyperphosphataemia. Its selective action on vitamin D receptors in the parathyroid glands allows effective modulation without significantly altering calcium and phosphorus metabolism. It has beneficial pleiotropic effects, including anti-inflammatory, antioxidant, and anti-fibrotic properties, reducing oxidative stress and systemic inflammation. It improves bone mineral density and reduces the risk of fractures in patients with advanced chronic kidney disease. Conclusions: Paricalcitol is established as a safe and effective therapeutic option for the management of secondary hyperparathyroidism.
