Abstract

Objective: To analyse the epidemiology, risk factors and pathophysiology of pterygium with an emphasis on the genetic mechanisms of p53 and TLR3. Method: A literature review was conducted in PubMed, SciELO, Science Direct and Google Scholar of articles published between 2009 and 2025 on aetiopathogenesis and prevention. Results: The global prevalence is 10.2%, concentrated in latitudes 0-40 degrees of the pterygium belt. Outdoor workers are the group at highest risk due to prolonged exposure to ultraviolet radiation. UV exposure causes mutations in the p53 gene, promoting excessive cell proliferation. The TLR3 gene recognises molecular patterns associated with cell damage, perpetuating fibrovascular growth. Diagnosis is made using a slit lamp. Initial treatment is conservative, with artificial tears. Conjunctival autografting is the surgical gold standard. Conclusions: Pterygium mainly affects workers exposed to ultraviolet radiation. The p53 and TLR3 genes are involved in its pathophysiology.
